The Community Health Advocate, Experienced will report to the Manager, Social Services Management, as described in the full job description. You will be responsible for conducting assessments regarding social drivers of health needs and communicating with members of the Blue Shield of California Promise Health Plan. The skilled Community Health Advocate will support the implementation of new services to fill in the gaps that have been identified in medical and community services.
Your Tasks In this Position:
Document member updates and progress notes in appropriate systems, submit timely reports, and provide recommendations for improved member outcomes File and make necessary follow-up on mandated reporting, such as Adult Protective Services, Child Protective Services, etc.
Perform additional duties as necessary QUALIFICATIONS Your Knowledge and Experience Bachelor’s degree in Social Work, Psychology, Biology, Public Health, Nursing, Community health, or Health-Allied field preferred OR equivalent experience in a related field Three years or more of experience working with people who need assistance with complex health and social issues Valid California driver’s license and proof of auto insurance required Proficient computer program skills, including Microsoft Excel, Outlook, Word, and PowerPoint Excellent oral and written communication skills Knowledge of health disparities in diverse communities Effective time management skills
This position has a pay range of: $ 36.68 for California, from $ 26.20.
