Full job description Seeking a team-oriented, energetic, compassionate individual with senior care experience. Customers and employees have given us a 5-Star Google rating, and we are family-owned and operated.

Summary of Position: The Care Coordinator is in charge of client outreach, placement services, assessment and care plan completion, client quality assurance visits, and client care coordination. They report to the Administrator.

The duties of this position will occasionally necessitate occasional caregiving during business hours.

On the phone or in person, discuss ABS services with potential customers and sources of referrals.
As needed, meet with clients or families to complete inquiries and intakes.
Complete client evaluations.
Evaluate clients to figure out what kind of services they need, such as activities, personal care, and services from outside sources. As required, coordinate and communicate with third-party service providers. Make a plan to address safety concerns and issues and evaluate and document them.
With clients and families, open new cases and complete service agreements and related forms.
Complete care plans for clients and contribute to the Generations program.
Complete and record customer quality assurance visits.
On the first visit, introduce the caregiver(s) to the client, family, or facility and make sure they understand the care plan by walking them into the case (such as a home or facility).
Give position administrations. Help clients and families locate and secure facilities that effectively meet their requirements. Work with facility staff to arrange placement. Establish contractual connections with facilities that provide placement services.
Liaise and communicate frequently with existing referral sources, in coordination with ABS Care Managers, to cultivate and maintain productive working relationships and generate ongoing referrals and business.
Establish positive working relationships and raise awareness of ABS’s service offerings by locating new and potential referral sources.
For the purpose of fostering productive relationships and disseminating information regarding ABS, schedule and carry out face-to-face visits, meetings, and in-services with existing and potential referral sources.
Participate actively in meetings with staff and marketing. Make suggestions for ABS’s marketing strategies.
Support ABS’s marketing campaigns and keep actively participating in them.
Every day, record marketing and referral source activities in the Generations system.
Work with the Staffing Department to ensure that cases in assigned territories are adequately staffed. Assign additional responsibilities.

Requirements for the Position: A minimum of one year of experience working for a homecare or home health agency or assisted living facility are required.
Understanding of the intake, admissions, and/or assessment procedures in a healthcare setting (such as an assisted living, homecare, or skilled nursing facility)
Experience working as a caregiver or a Certified Nursing Assistant (CNA) and/or a demonstrated comprehension of the best ways to provide care.

ability to effectively represent ABS in person and over the phone to clients and the general public.

The position calls for 50-65% travel. We reimburse you for mileage.
